Essential Functions:

The duties identified below are the essential functions of this position. Reasonable accommodations may be able to enable individuals with disabilities to perform the essential functions.

  • Receive all referrals into the Community Partners program
  • Contact all new referrals within 5 business days
  • Document all calls and any contact with all referrals into Community Partners database
  • Input all demographic information for all new referrals into the Community Partners database. Give new referrals once contacted to the Community Partners Supervisor to assign to Home Based Family Specialist.
  • Email each Home Based Family Specialist the new referral client number for all new referrals assigned.
  • Input all new Community Partner referrals into the monthly stats document. Make intake packets for all 7 counties and for Safe Sleep program.
  • Complete counseling and client supply purchase orders per managers request.
  • Contact vendors for W9 information.
  • Complete and reconcile monthly stats document with Community Partners database for supervisor.
  • Attend staff meetings and mandatory training.
  • Input all closed file numbers into the Excel spreadsheet.
  • Serve as a backup for any Home Based Family Specialist and Safe Sleep Specialist as needed.
  • Administrative Assistant duties for Community Partners, Safe Sleep; including front desk coverage per manager's request.

QUALIFICATIONS:

Must be 19.5 years of age or older.

To perform this position successfully, an individual must demonstrate regular, predictable attendance and be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge and skill required. The individual must be able to successfully pass background checks.

Education/Certification/Licensure:

  • Associates degree in sociology, psychology, education, child development, or other related field; Bachelor's degree preferred.

Experience Required:

  • 2-3 years previous field experience

Knowledge and Skills Required:

  • Excellent communication skills, both written and verbal
  • Strong math skills
  • Attention to detail
